Bugaboo Recalls Cameleon3 Strollers Due to Fall Hazard

Bugaboo Americas has issued a nation-wide recall involving over 10,000 units of the Cameleon3 stroller model, due to a potential fall hazard caused by the carrying handle detaching while the removable seat is being carried. Bugaboo has received 16 reports of handles breaking, none of which have resulted in injury. Strollers included in the recall have serial numbers from 19010 11153 00001 to 19010 51248 00215. Serial numbers are printed on a horizontal bar of the stroller’s chassis beneath the seat. 9,200 of these were sold in the U.S. and 960 in Canada. In order to remedy the situation, Bugaboo is recommending consumers immediately remove the carry handle from the stroller’s bassinet or seat and contact Bugaboo for a free replacement handle. While awaiting the replacement handle, consumers can continue to use the seat or bassinet when attached to the chassis but should not attempt to use the seat or bassinet separate from the chassis.

3M Recalls Filtrete Room Air Purifiers Due to Fire Hazard

3M has recalled around 10,000 of its Filtrete Air Purifiers yesterday, due to potential fire hazards caused by overheating. The air purifiers are white, made of plastic and plug into the wall. They measure about 19 inches tall by 8 inches wide with a 13 inch tall by 4.5 inch wide air filter. The two recalled models are Ultra Quiet, number FAP00-RS, and Maximum Allergen, number FAP00-L, which was sold only at Lowe’s stores. The products serial numbers begin with E, F, G, H, I or J and the model and serial numbers are located on the bottom of the product. Two incident reports have been filed so far: one involved the product overheating while the other actually caught fire. No injuries or damage was incurred in either case. If you have one of these products in your home, please unplug it immediately. Also, 3M is recommending consumers contact the corporate office at (800) 388-3458 from 7 a.m. to 6 p.m. CT Monday through Friday to receive a prepaid shipping box so that they may send the recalled product and recieve a replacement, free of charge. Among the retail locations currently selling these purifiers are:

Ultra Quiet at Ace Hardware, Bi-Mart, Do It Best, Fred Meyer, Handy Hardware Wholesale, Nuthouse, Orchard Supply, Orgill Bros., Petco, Rite Aid, Strosniders, Theisen Farm & Home, True Value Hardware stores nationwide, and online at Amazon.com and others. Maximum Allergen sold only at Lowe’s stores. The FAP00-RS model was sold from November 2008 through January 2013, and the FAP00-L model was sold from October 2012 through January 2013, both for about $60.

KC Man Loses Both Legs in Tire Shredder

Rescue personnel were dispatched to a downtown Kansas City business, ABC Tire Recycling, where a man was said to have fallen into one of the tire shredders on the premises. Upon arrival, they found that the man had been pulled from the machine by coworkers and was in critical condition. The victim was performing maintenance on the shredder when it spontaneously powered up and pulled him in. He was taken to a local hospital and has been confirmed to have survived, despite both main arteries being severed. While no new reports have come in today, authorities say both legs had to be amputated upon arrival at the hospital.

The Occupational Safety and Health Administration is investigating the case and hope to determine whether or not the accident was caused by a product malfunction.

$14.5M for boy left brain dead in metal bat lawsuit

A New Jersey teenager left brain-damaged after being struck by a line drive off a metal bat while he was playing in a youth baseball game will receive $14.5 million to settle his lawsuit against the bat manufacturer, Little League Baseball and a sporting goods chain.

The settlement of Steven Domalewski’s lawsuit was announced in state Superior Court Wednesday morning in Passaic County. The boy, now 18, lives in Wayne, N.J. His family had claimed the metal bat was unsafe because baseballs could carom off it at much faster speeds than wooden bats.

The boy was left permanently brain dead as the result of his brain being without oxygen for 15-20 minutes.

Bumbo recalls four million baby seats

A South African manufacturer is recalling about four million baby seats that were sold at major retailers including Target Corp., Wal-Mart Stores Inc. and Toys “R” Us Inc. after reports that at least twenty-one babies fell from the seats and fractured their skulls, the U.S. Consumer Product Safety Commission said.

Bumbo International Trust is recalling its Bumbo baby seats after receiving at least 84 reports of infants falling or maneuvering out of their seats.

The reports followed an earlier recall in October 2007 of the same seats. At that time, Bumbo recalled one million of the seats in order to provide additional warnings against use on raised surfaces.

Most of the incident reports after 2007 were related to use on raised surfaces, while about 34 incidents were of infants who fell or maneuvered out of a seat used on the floor or at an unknown elevation, at times resulting in injury.

The bottom of the molded foam seat is round and flat, with a roughly 15-inch diameter, the regulatory agency said. The seat has legholes and the back wraps around the child. The front of the seat has the word “Bumbo” in raised lettering, with an image of an elephant.

Consumers are urged to stop using the products immediately, until they order and install a free repair kit, which includes a restraint belt and safe-use instructions.

The seats were sold by Babies “R” Us, Sears, Target, Toys “R” Us, Wal-Mart, and other toy and children’s stores nationwide, as well as online, from Aug. 2003 through Aug. 2012 for between $30 and $50.

Lawsuits filed in fatal St. Louis tent collapse

The St. Louis Post Dispatch is reporting that two of the nearly 100 people injured in the April 28 storm accident at Kilroy’s Sports Bar filed lawsuits today against the pub and the company that leased and installed a tent that collapsed.

These are the first of several expected lawsuits to be filed against Kilroy’s and the company that leased and installed the tent.

Missouri residents infected with salmonella from recalled Diamond Pet Food

The total has now risen to 16 individuals that have become infected with Salmonella from at least 11 brands of dry dog food manufactured at a Gaston, S.C., plant operated by Diamond Pet Foods of Meta, Mo. They include the store brand pet food sold by Costco, Kirkland Signature and Kirkland Signature Domain.

Lab tests have confirmed that those sick were infected with a rare strain of salmonella Infantis, which was detected only after a routine test of dry dog food revealed contamination, a report from the Centers for Disease Control and Prevention said.

Although three individuals are from Missouri, people have been infected in eight other states including North Carolina, Pennsylvania, Alabama, Connecticut, Michigan, New Jersey, and Virginia.

People can become sick by handling the tainted pet food or through contact with an animal that has eaten the contaminated food.

Inflatable pool slide death prompts recall

In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Wal-Mart Stores Inc., of Bentonville, Ark. and Toys R Us Inc., of Wayne, NJ are announcing the recall of about 21,000 inflatable Banzai in-ground pool water slides. During use, the slide can deflate, allowing the user to hit the ground underneath the slide and become injured. The slide is also unstable and can topple over in both still and windy conditions and carries inadequate warnings and instructions.

The CPSC is aware that a 29-year-old Colorado mother died in Andover, Mass. after fracturing her neck going down a Banzai in-ground pool water slide which had been placed over the concrete edge of a pool. The victim hit her head at the bottom of the slide because it had partially deflated.

The CPSC and the retailers are aware of two other injuries which have occurred in a similar manner, including a 24-year-old man from Springfield, Mo. who became a quadriplegic and a woman from Allentown, Pa. who fractured her neck.

The recall involves Banzai in-ground pool water slides designed for use with in-ground pools. The vinyl slides have a blue base, yellow sliding mat and an arch going over the top of the slide. By connecting a hose to the top of the slide, water can be sprayed on its downward slope. The words ‘Banzai Splash’ are printed in a circular blue, orange and white logo, shaped like a wave on either side of the slide.

The recalled slides, which were manufactured in China by Manley Toys, Ltd, were sold at Walmart and Toys R Us nationwide from January 2005 through June 2009 for about $250. The recalled slides have the barcode number 2675315734 and model number 15734. Both the barcode and model number appear on the original packaging but are not on the actual slide.

CPSC urges consumers to immediately stop using the product and return it to the nearest Walmart or Toys R Us for a full refund. Consumers can also cut the two safety warning notices out of the slide and just return that portion. St. Louis worker falls from crane while working on new Mississippi River bridge

The AP is reporting that St. Louis rescue crews are searching for a crane operator who fell into the Mississippi River while working on a new bridge at St. Louis. The St. Louis Post-Dispatch reports that the crane broke Wednesday morning and fell over in the water. St. Louis Fire Department rescue crews went to the scene. The worker’s life jacket was found, but not the man.The worker was helping to build the new $640 million bridge.

As spring begins and the economy contrinues to strengthen, many construction workers will be returning to work. Large projects and developments can often lead to worker safety hazards or require working with dangerous or defective equipment. NHTSA expanding probe into stuck Ford throttles.

The AP (3/13, Krisher) reports the National Highway Traffic Safety Administration is “investigating sticky accelerators in as many as 1.9 million Ford Taurus and Mercury Sable sedans.” The probe, which began last week has expanded “to include cars from the 2001 through 2006 model years.” In the 14 complaints of Taurus sedans accelerating on their own, the agency hasn’t heard “of crashes or injuries because of the problem.” The agency suggested that “a faulty cruise control cable” that “can detach and hold the throttle open” could be the problem.

The Detroit News (3/13, Shepardson) reports NHTSA spokeswoman Lynda Tran said the agency “is carefully evaluating all available data and will share any findings upon conclusion of its investigation.” Adding that the “request ‘is inquiring about a broader population in order to obtain a basis for comparison,’” she also said that “it isn’t uncommon to look at similar vehicles as part of a preliminary investigation.”

Car Wreck in Springfield Ejects Baby and Car Seat

A two-vehicle crash in Springfield sent a 1 year old to the hospital after her car seat was ejected.

Cindy Wells, 45, of Springfield, said she was driving south on Kansas Expressway and making a left-hand turn onto Hovey Street. Wells, who was driving a 1986 Bronco, said she was hit by another car.

The baby was in a forward facing car seat but was thrown out of the back of the vehicle during the wreck.

“The only thing we could figure is that the pressure on the seatbelt caused the lock to disengage,” Wells said.

Fortunately, no one in the accident was seriously injured including the baby, who was transported to the hospital with minor injuries.

Goodyear Tire Recall

Goodyear announced this week that it would recall 41,000 of its wrangler silent armor tires. These tires were produced in 2009 and in 2011 at least two people have died as a result of a small tear in the tire

“A small number of tires within this population may experience a partial tread area separation under certain severe usage conditions,” Goodyear said in a February 22 letter to theNational Highway Traffic Safety Administration. “Use of these tires in severe conditions could result in partial tread separation which could lead to vehicle damage or a motor vehicle crash.”

Exploding fuel gel in firepots raises safety concerns

A Lenexa woman knew the dangers posed by gel fuels long before a warning this week from the Consumer Product Safety Commission.

Last October, Pam Polson and her family sat on her sister’s patio catching up on each other’s lives.

Her brother-in-law, Steve Michnick, began to refill a Napa Firelite firepot with a gel fuel called NAPAfire. All Polson can remember are the flames landing on her.

“It was horrible,” she said. “I mean, I was flaming. One minute I was sitting doing nothing, and the next thing I know these flames shot out and landed on me and I was immediately engulfed in flames.”

The Consumer Product Safety Commission issued a warning on gel and other fuels used in firepots, tiki torches and other products. Spokesman Alex Filip said the agency had received multiple reports that are being investigated, but the focus of the investigation was on the fuel.

“Because it’s a jelly-like substance, it will get onto clothing or skin and when it catches fire, just splashing water or smothering it won’t always kill the flames,” Filip said. “You need a dry chemical extinguisher to put it out.”

Filip said the gel fuel has only been available on the market a couple of years, to his knowledge. He said it would take time for scientists to understand how to best deal with the product and to review its safety labels. Already, though, retailers are pulling the product.

Filip said the most important thing is for the public to be aware that the products are hazardous.

“It looks like the flame has gone out when it actually hasn’t,” Filip said. “When people pour the fuel on when the fire is not completely out, it’s like pouring fuel onto a fire.”

Girl dies after fall from Ferris wheel

An 11-year-old girl died Friday after falling from a Ferris wheel while on a class trip to an amusement park in New Jersey, police said.

Abiah Jones fell at Morley Piers Mariner’s Landing Pier about 12:30 p.m., according to the Wildwood Police Department.

First responders gave her first aid and took her to Cape Regional Medical Center, where she was pronounced dead about 1:15 p.m., according to police.

The girl was riding one of the park’s main attractions, “The Great Wheel.” At 156 feet, it’s one of the tallest wheels on the East Coast, according to Morley Piers spokeswoman Lindsey Young.

The cause of the girl’s fall was unknown and the Wildwood police and the Cape May County Prosecutor’s Office continue to investigate.

The Carnival Amusement Inspectors of New Jersey were also on the scene Friday afternoon conducting their own investigation, police said.

It is the first time someone has fallen from the ride since it opened in 1985 and the park has suffered no other guest fatality, Young said.

The ride was inspected by the state in March and passed, Young said.

Man dies after being hit in head with split rim wheel

A man working on a tractor was killed last week at a local salvage yard when a split rim wheel struck him in the head.  The man was working at a salvage yard when a tire exploded off the rim of an industrial tractor, cutting his head open and causing severe brain trauma.  Split rim wheels are known to have random explosions, given the inflation pressure, according to the Occupational Safety and Health Administration.

Horse feed recalled by manufacturer

A Missouri-based horse feed manufacturer has announced the voluntary recall after it could contain medication that could be fatal to horses.

The horse feed was distributed in California, Nevada and Oregon by Manna Pro Products. The recalled product is called Family Farm Complete Horse 10 horse feed with a lot number of 1006.

It was found that the product may contain potentially dangerous level of Rumensin. The feed was distributed for two weeks in the month of January. The company has urged customers to stop using the product.

Gingerbread houses sold at Whole Foods Market recalled in 22 states

Gingerbread houses that were produced by Rolf’s Patisserie and sold at Whole Foods Markets have been recalled in 22 states.

The products are a part of a larger recall from the baker when it was found that the products may be connected to outbreaks of Staphylococcus aureus food poisoning. Whole Foods Market outlets sold the ginger bread houses in 22 states including Missouri, Nebraska, Colorado, New York, Oklahoma, Rhode Island, Illinois, Indiana and Wisconsin.

Whole Foods made a statement that there has been no illnesses reported from people who bought the house from their store. The Food and Drug Administration advises anyone who bought this product to throw it away.

Senate passes new Food Safety Act

After the amount of deaths last year due to food safety, the concern has been raised significantly. The U.S. Senate has passed the S510 Food Safety Modernization Act on November 30 and has drawn support from party lines.

The goal of the act will prevent contaminated food from being on the market by bringing into effect multiple new regulations.

The regulations will require new food safety plans to prevent these contaminated foods from entering the market and will help to raise the confidence of consumers. The regulations will make it a necessity for grocery stores to make all of the information on the recalled foods visible.

Whole Foods announces recall of two dairy products

Whole Foods Market has announced the voluntary recall of two different dairy products after possible Listeria and Staphylococcus aureus contamination.

The two products include Morningland Dairy and Ozark Hills Farm Raw Goat Milk MIld Cheddar Cheese which were sold at stores in the North Atlantic region in Connecticut and Massachusetts.

The recalled products are sold in the cheese department and have the Whole Foods scale label on them. Customers who have purchased these cheeses may return them to the store for a full refund. The FDA, the Missouri Milk Board and MorningLand Dairy are currently investigating the contamination.

Eggs recalled due to possible salmonella contamination

Several brands of eggs have been recalled due to a possible salmonella contamination at two chain stores, Albertsons and Ralphs.

The recall effects eggs that were distributed to food wholesalers, food service companies and distribution centers in multiple different states such as Nebraska, California, Illinois, Missouri, Wisconsin and Iowa.

The recalled eggs were packaged by Wright County Egg of Galt and it has been said that the company is working cooperatively with the Federal Drug Administration during the investigation.

Natural Balance dog food recalled in multiple states

Natural Balance has pulled Sweet Potato and Chicken dog food from the market due to possible salmonella contamination. The company took the food off of the market after a random sample tested by the Federal Drug Administration tested positive for the bacteria that can cause food poisoning.

The sample that was tested had a ‘best by’ date of June 17, 2011, according to the FDA. The 5-pound and 28-pound bags of the Sweet Potato and Chicken dog food are effected in the recall. There have been no reports of illnesses due to the recalled product.

The company also hired an independent lab to test the recalled products. The product has been distributed in pet specialty stores in 26 states including: Alaska, Arizona, Illinois, Minnesota, Missouri, Texas, Utah and Oregon.

Fire hazard causes Whirlpool to recall 1.7 million dishwashers

Whirlpool Corp with the U.S. Consumer Product Safety Commission have voluntarily issued a recall for 1.7 million dishwashers due to a fire hazard.

The company has received 12 reports of heating element failures that have resulted in fire which has caused serious fire damage to the dishwashers and in one case, extensive damage to the whole kitchen.

Other brands in the recall include Jenn-Air, Crosley and Amana. The dishwashers were sold from February 2006 to April 2010.

Horse and poultry feed recalled

Purina Mills, LLC and the Food and Drug Administration have issued a voluntary recall of Strategy Horse Feed and Layena Poultry Feed due to potential health hazards to animal consumers.

According to officials, the recall covers the 50-pound bags of food and has been implemented after the findings of metal fragments in the bags. Five consumer complaints have been reported. Consumers who have bought the 50-pound bags of feed have been urged to bring the product back to the dealer in order to receive a replacement product.
